About Recommence Recovery

What Is Case Management?

A case manager provides hands-on, non-clinical support that helps individuals stay stable while navigating recovery or mental health challenges. While therapists and psychiatrists focus on treatment, a case manager helps organize and support the day-to-day pieces of life that make treatment effective.

Case managers work alongside the client and their treatment team to ensure care is coordinated, communication is clear, and support is consistent. This includes helping clients follow through on recommendations, maintain structure and routines, and address concerns early—before they become crises.

The role of a case manager is to bridge the gap between appointments and real life, offering practical guidance, accountability, and coordination during times of transition or increased vulnerability.

  • Support with routines, structure, and accountability
